DOT Physical vs DOT Drug Test: What's Included?

Short answer: No — a DOT physical does not include a drug test. They're two separate requirements that often get confused because both involve a certified clinic.

The DOT physical

The DOT physical (officially the DOT medical exam) is performed by a certified Medical Examiner and results in a Medical Examiner's Certificate (MEC, or "med card"). It checks vision, hearing, blood pressure, urine for sugar/protein (not drugs), and overall fitness to operate a commercial motor vehicle.

The DOT drug test

The DOT drug test is a 5-panel urine test for marijuana, cocaine, opiates, amphetamines, and PCP. It's collected at a separate certified collection site, sent to a SAMHSA-certified lab, and reviewed by a Medical Review Officer.

Same urine sample?

No. The urine sample collected during the DOT physical is a dipstick check for glucose and protein. It is never sent to a lab for drug analysis. A DOT drug test requires its own chain-of-custody collection.

When you need each one

  • DOT physical: Before driving and at least every 24 months thereafter (sometimes more often).
  • DOT drug test: Pre-employment, random, post-accident, reasonable suspicion, return-to-duty, and follow-up.

Can you do them at the same clinic?

Often yes — many clinics offer both services, and bundling the visit saves time. But they are billed and recorded as two separate services.
